I painted and glued a scrabble tile holder onto the door to hold the chalk.
The raw door.
The chalkboard is mounted at the back door. The hardware on either side of the door are the original door knob plates.
I copied part of a poem for now but the fun thing about a chalkboard is that it can always change.
I mounted a latch to the door to hang the clock from.

I am pleased with the way my chalkboard door looks hanging at my back door. I may add some more hardware sometime down the road but for now, like the clean simlplicity of it.
